Provides regular contact to alleviate loneliness for adults 55+ following discharge from hospital

Detailed Description

A Home Independence Befriender will visit an older person in their own home on a number of occasions usually over a 2-4 week period following discharge from hospital and professional support from the Home Independence Support service team. Length of commitment: Ongoing but rotating - providing support to particular individuals for a 2-4 week period following discharge from hospital You will provide company and conversation to alleviate isolation and potentially assist in identifying opportunities to engage in wider social groups or accessing technology.
